That on the 6th day of May, 1985, PRACTICE MANAGEMENT ASSOCIATES, INC. WHICH WILL DO BUSINESS IN CALIFORNIA AS FLORIDA P.M.A., INC., a Corporation organized and existing under the laws of Florida, complied with the requirements of California law in effect on that date for the purpose of qualifying to transact business in this state. I further certify that said corporation was forfeited on May 6, 1985, pursuant to the provisions of the California Bank and Corporation Tax Law, more Particularly Section 23302 of the Revenue and Taxation Code, and was revived on March 22, 1988, as reported by the Franchise Tax Board. That the records relating to this corporation are now in the active files, reinstatement having been effected. In Witness Whereof, I execute this certificate and affix the Great Seal of the State of California this 27th day of October, 1989
